Online Forms & Instructions

At South Georgia Crematory, we aim to make our services as professional as possible.  This starts with our online forms to make each step as effortless as possible.  Both steps must be completed before any remains are received by the crematory.  Please complete STEP ONE to activate your account now, so that when you have a cremation, you will only have the second step to complete.

STEP ONE

As a New Client, you will need to fill out two online forms Request for Credit & Master Agreement and provide all required documentation as stated in the New Customer Account Requirements.
To avoid any handwriting discrepancies, both of these forms can be filled out online and printed in your office.  All you need to do is mail the originals of these two signed forms along with the other required documentation to the business office mailing address.  Once you have completed STEP ONE and registered your company, you will not have to complete these forms again.  Updated proof of insurance and business licenses will be required periodically.

STEP TWO

Now that you have activated your account, you will only need to complete two forms to arrange a cremation.  Like the forms in step one, these forms are online for your convenience and to avoid handwriting discrepancies.

    • The original completed Cremation Case Sheet must accompany the remains when they are delivered to the retort.
    • The original Authorization for Cremation should be kept by the funeral home and a COPY must accompany the remains when delivered to the retort.
    • Complete a state of Georgia permit for the disposition of human remains. Crematory should receive a COPY to accompany remains.

After you complete these forms, please call the retort location 229 386 2619 to make a drop off appointment.

With the completed forms in hand, deliver the remains in an approved cardboard container to the crematory.  We will provide a replacement container as you depart.
